Middle School Wellness Calendar Assignment
Each month all middle school students will receive a “Wellness Calendar”. The purpose of the Wellness Calendar assignment is to help you realize some of the positive healthy habits you have developed and to remind you of some habits you might work on.
Before a person can begin to make lifestyle/habit changes it is important to know exactly what your habits are. Each day in your science class you need to open to your wellness calendar in your binder and place a check or an X in the spaces of the behaviors you were successful in achieving in over the previous day. If you did not achieve a certain behavior put nothing. At the end of the month you will add up the total number of successful days you had for each behavior.
The wellness habits or behaviors include nutrition, rest, exercise, leisure activity, and hygiene among others. If you have a habit you would like to see addressed call, write a note or e-mail me.
Your responsibilities are:
· Make sure you know what is expected for each behavior (on back of calendar, ask Mr. Plouf or parents)
· Review the success for each behavior in science class each day
· Have your mom or dad sign your calendar each week
· Turn your calendars in on each due date.
· Add up your totals for each behavior for the month
The success of this assignment will be determined by your efforts.
Your grade for this assignment is based on you paying attention to and keeping track of your behaviors with the wellness calendars. If you misplace your calendar see Mr. Plouf as soon as possible to get another.
