You can return the completed registration form with payment three different ways:

  1. Drop off form and payment at the school office

  2. Mail to:  Connie Anastasi, c/o St. Monica School, 4320 87th Ave SE, Mercer Island, WA  98040

  3. If paying with a credit card you can email completed form directly to Connie Anastasi at canastasi@stmonicasea.org
