Permission Form
The “Science Kit Program” is for Kindergarten through 5th grade students and features 50+ exciting experiments for your little scientist to explorer at home! Each kit is a self contained science experiment that teaches science concepts in a fun, hands-on way.
Each kit includes a Scientific Observation form which may be completed along with the experiment. There will be fun prizes for scientific feedback awarded!
The kits will be distributed on Tuesdays, you will have 2 full weeks to complete and return your student’s kit. Once you’ve enrolled your student in the program, a list of kit choices will be emailed to you.
In order for your child to be able to complete the kits at home, a permission slip MUST be on file. Please fill out the form below and either email it to Monica Tracey (mtracey@wolfenet.com) or place it in the Science Kit folder in the school office.
Note: Your child may join the program at any time during the school year.
